Like Brunanas said, most likely rolling to close to the wall, thus getting legs stuck in the air. Then they can't finish rolling and they can't roll back. Then they panic because they feel vulnerable or know they can't stay laying like that and injure themselves. Even if they don't freak out, being stuck in that position can cause internal damages and lead to severe colic and death that way. Its a horrible situation. And it can happen outside too, if they choose to lay or roll in a weird spot. Sheep, cattle, goats and probably other animals can get cast as well.
Oddly enough, a few days ago I saw a post on some walking/countryside FB page from England advising walkers that if they saw a sheep on its back, to try and roll it onto its side so it could get up. So it can happen even in open country. Didn't Nashoba's Key fatally injure herself like this?

Thanks. I remember it was reported that she had "kicked her stall" but I couldn't remember if being cast was part of it.
I ran marathons. I saw the Taj Mahal by Moonlight. I drove Highway 1 in a convertible. I petted Zenyatta.

I can’t believe there were not cameras in her stall! What a loss.
That’s a bit harsh to the staff who have been working tirelessly to keep her alive since November. Does it say anywhere that they don’t have cameras?

Even if there are cameras in her stall it wouldnt have stopped her. The only thing that could stop her in the 3 seconds it wouldve taken for her to roll over and start sliding her hooves against the wall wouldve been a person standing there 24/7 watching her. These things happen so fast when a horse panics. Sometimes a nice senior or docile warmblood will stay still against the wall or fence he or she is cast on until help arrives with lunge lines to flip them over or a chainsaw to cut the fence, but a Thoroughbred filly who has been on stall rest for 4 months?

Ziggy you've never been around a horse on stall rest have you? Hell even foaling barns with cameras can't do crap when something bad happens sometimes

MRH lost Show Your Cards to being cast on his stall. He broke his shoulder and couldn't be saved.

I saw the news about Echo Zulu while I was at work and just sat there, stunned. Poor girl.

There's a tiktoker with a horse who casts himself all the time. I think she tried the bedding thing but she still did it

This is sad and unfortunate but I think it falls under the category of Stuff Happens. I had that gelding Anniversary Year almost 19 years. Once I got some land and could keep him with us at home, we opened the stall doors and just let him roam. He always had the stall if he wanted it, his hay and feed would be given in there but if he wanted to get out he could. Border Run had the same set up as they were turned out together. Now I know this isn’t always feasible and there are still a million other ways a horse can hurt themselves—especially a TB—but they stayed much more even keeled that way. The two QH we have now act like they’ve been given ACE every morning.

She kicked the stall wall and broke her hock. Think it shattered her tibia in the process, and was not able to be saved.

You can bank the bedding along the stall wall really high, like 3-4 feet of shavings, to create a slope that keeps a horse from rolling into the wall. Someone on a budget though may not have the extra money to double the amount of bedding they use in every stall. Or youll still have a horse roll into the door of the stall that wasnt banked with bedding - which i have had happen with a senior.

A good option is putting one or two strips of either 2x4s of wood or rubber on the walls of the stall, around waist height. So if the horse rolls into the wall, it has a ledge for its hooves to catch so it can push itself back over.

I had a share in show your cards. I was absolutely shattered when that happened. Echo zulu brought back memories I'd buried
